If admitted to the Emergency Room or receiving care from a hospital or specialist, Dr. Rafatjah will speak with admitting doctors and brief all parties on your case. Your medical records will be sent to facilitate seamless transition of care.

Sadly, medical errors are the 3rd leading cause of death in the U.S., largely due to ineffective communication between healthcare systems, specialists and doctors. We help ensure that each medical decision is informed and intelligent.

We follow your outside care throughout the entire process in order to greatly reduce unnecessary procedures, medications and room for error.
